item. Please download the ORDER FORM and phone or fax in the order to CSC, Inc directly (Phone & Fax #: 757-631-8837).We
accept personal or corportate checks via mail and credit cards via fax. Mastercard or Visa credit cards are
accepted methods of payment. Hospitals may mail or fax a Purchase Order to CSC, Inc and an Invoice will
accompany the shippment. If you use a credit card, your order will ship immediately. If you pay by personal check, your order
will ship 5 business days after we receive your check.
